HIPAA Rules
The Health Insurance Portability and Accountability Act (HIPAA) was enacted by the US Congress in 1996. Title I of HIPAA protects health insurance coverage for workers and their families when they change or lose their jobs. Title II of HIPAA, known as the Administrative Simplification (AS) provisions, requires the establishment of national standards for electronic healthcare transactions and national identifiers for providers, health insurance plans and employers. The Administration Simplification provisions also address the security and privacy of health data. The standards are meant to improve the efficiency and effectiveness of the nation's healthcare system by encouraging the widespread use of electronic data interchange in the US healthcare system.

Important Notice: This notice is to advise you that as a member of the Plan, information created, maintained or received about you may be subject to electronic disclosure, and in keeping with appropriate authorizations from you or your legal representative or as otherwise required by law. As always, our organization remains committed to ensuring the privacy and security of your protected health information.
We respect your privacy as a visitor to our website. The information collected during your visit is limited and only used to help us support and enhance our relationship with you. Your information may also be used to improve our services, your healthcare, and your online experience.
The information we use to help us better understand our visitors includes domain names but not email addresses. Email addresses are collected only when you initiate communication with us via email or provide us the information. In order to help us improve services and content, we may collect information on what pages are being accessed or visited. You provide any other information that we gather via surveys, forms, and registrations.
Personal information that you provide is limited to the specific use for which you provided it. That means that your postal and email addresses and telephone numbers will only be used to contact you regarding a request you submitted online.
Your information will not be shared with any organization outside of our organization without your consent, except as required by law.

Make a HIPAA Complaint
You may use either of the following two methods to submit a HIPAA complaint:
- Visit the Compliance HelpLine — our online compliance portal; or
- Call 888.484.6977 (toll-free within the United States, Guam, Puerto Rico and Canada), any time.
After you complete your report, you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, you can use your report key and password to check your report for feedback or questions.
NOTE: The Compliance HelpLine is NOT 911 or Emergency Services: Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

Information Collection & Use
For purposes of this site, we may collect and use your email or phone/text number that you provide in order to respond to your inquiry or to send you information about a service or resource that you expressed interest in.
You may also decide to use your email or text number to ask us questions and provide comments about our website or services. You are encouraged to provide this feedback. We do share these messages with individuals in our organization who can appropriately respond; we may preserve the content of your message along with our response.

Confidentiality of Personal Information
Baylor Scott & White Health Plan (BSWHP) uses reasonable efforts to protect the privacy and confidentiality of your personal information, including encrypting personal information through our self-service portals. In addition, employees are subject to confidentiality and compliance policies relating to personal information.
When communicating personal information through the Plan self-service portals, you will be required to log on to the secure portal, using credentials provided during registration. Your password is your private key to access your account and personal information. To maintain the confidentiality of your account, you should never share your password with anyone. In addition, you should change it periodically and you should select a password that is difficult for others to identify.
Promotions, Information or Other Offers
We may, from time to time, send you information or offers regarding Plan products or services, or others in which we believe you might be interested. These offers will include an option that you may execute in order to discontinue similar contacts.
Children
This site is not intended for use by children (persons under the age of 18).
Links to Other Sites
The Plan may provide links to other websites. Please be aware that once you leave our website and enter another website, you may be subject to different privacy practices and policies over which the Plan has no control and for which the Plan is not responsible. Please use prudence in utilizing such links and in disclosing any personal information on those sites.
Changes to Internet Privacy Statement
The foregoing Internet Privacy Statement is effective as of October 11, 2021. The Plan may change this statement periodically without notice. Changes to our Internet Privacy Statement will be noted here so that you can be informed about the privacy protections we provide. This statement is not intended to and does not create any contractual or other legal rights in or on behalf of any party.
