Welcome to BSW Be Well™ with Diabetes — a program for Baylor Scott & White Health Plan members with diabetes. Our goal is to help you manage your condition and improve your health.
What's in it for you?
- If you sign up and complete the initial requirements, your copays for diabetes medications and testing supplies will be waived for up to 12 months.
- You will be eligible for a free glucometer and a free app to track your blood sugars.
- You will be enrolled in the BSW Be Well™ Diabetes program and BSW Be Well™ Coaching, with eight virtual coaching sessions.
- A clinical pharmacist will review your medication list for drug-related problems and will be available to meet with you for any questions you may have about your medications.
How does that help?
With access to no-cost diabetes medications and monitoring systems, you won't have to put your health on hold if money gets tight. The BSW Be Well™ Coaching program can identify simple changes that will improve your health—and provide tips on how to make those changes as easy as possible. Having a pharmacist available to answer your questions will help you stay better informed about your medications and how they may be affecting your health.
Are you eligible?
If you are a:
- Member of Baylor Scott & White Health Plan
- AND a member of Commercial Large Group (excluding HDHP)
- AND you have Type 2 Diabetes
- AND you're 18 years old or older
...then you are eligible to participate in the program.

How does the program work?
If you have a non-BSWH provider, please have them fax your labs and documentation of provider visit to 254.298.6110 or email to BSWHPDiabetesProgram@BSWHealth.org.
