Comprehensive Care Management
Free Assistance with Chronic Health & Access to Care
Living with a health condition can be challenging. To make it a little easier, the Baylor Scott & White Health Plan (BSWHP) Case Management team provides care management services to employees and dependents of Baylor Scott & White Health. Through care management, there is a multidisciplinary team available to support you in managing your health, assisting with navigation and coordination of care. Additionally, care managers work with members, families and providers to create plans to meet the ongoing complex care needs specific to each member. Care managers advocate for members, answer questions, provide education and assist members with setting goals.
A care manager may reach out to you if you have been identified as having a chronic health condition such as asthma, diabetes, COPD or a variety of other conditions that benefit from focused care. Care managers may communicate with your physician to facilitate conversations about your health concerns. At the core of care management is the goal to improve members' health and quality of life in addition to getting the greatest value from their health plan.

BSW Maternity Care Management
Our Expecting the Best® Case Management Program is available to all medical plan participants who are pregnant, up to 1 year postpartum and infants up to 1 year of age. Participants can receive additional support from RN and/or SW Care Managers, including scheduled screenings tailored to where they are in their maternity journey, postpartum depression and maternal behavioral health specialty support, coordinated care and resources to promote a healthy mom and baby. There is no additional cost to members for this program. Benefits include:
- Education and support during pregnancy and after delivery
- In-home support for high-risk conditions (such as diabetes, high blood pressure and severe nausea)
- Screening for early identification of perinatal depression
- Newborn health education
- Breastfeeding best practices
To learn more about or sign up for Expecting the Best® Case Management send an email to HPEmployeePlanCaseManagement@BSWHealth.org with you name, health plan member number, date of birth, estimated due date (or when delivered) and any needs you have. All members automatically have maternity benefits. To learn more about your benefits:
- Log into the MyBSWHealth member portal to view your benefit information.
- Call the customer service number on the back of your ID card.
NOTE: Welcoming a new child is an exciting time with many things to remember, but there is one important step you should take as soon as possible after delivery - add your newborn to your benefits within 30 days of birth. Visit and log into MyPeoplePlace.com to submit a life event and add your newborn to your benefits.
Behavioral Health Care Management
Our Behavioral Health Case Management program is available to all medical plan participants. It is designed to help individuals with mental health or substance use disorders access and coordinate the care and resources they need to manage their conditions and improve their overall well-being.
Behavioral Health Case Managers are licensed Social Workers who work directly with members or their legally authorized representatives to assess individual needs, create personalized service plans and connect members with appropriate health plan benefits and community resources. They help members navigate covered services, provide ongoing support and follow-up and offer education about available benefits. Case Managers also assist with finding doctors and behavioral health providers, locating affordable medications and accessing community resources to address financial challenges.
This program is client-centered and goal-focused, with the primary aims to:
- Improve daily functioning
- Promote recovery
- Reduce hospitalizations or crisis episodes
- Enhance overall quality of life
