Section 204 (of Title II, Division BB) of the Consolidated Appropriations Act requires that information be submitted about 2025 prescription drug and health care spending. This data submission is called the prescription drug data collection (RxDC) report.
For Baylor Scott & White Health Plan to submit RxDC data on your behalf, we must collect certain information that is not contained in our system. Your completion of this form by April 15, 2026, will ensure we can submit the report on your behalf. Submissions after April 15 will not be accepted.
